Update Your Child’s Information for the 2026-27 Year

As we begin a new program year, please help us update your child or youth’s information and release forms for the 2026-27 school year. Please complete one form per child/youth participating in nursery, children’s ministry, or youth ministry activities during the school year. Paper copies available on request.
